Request for Exception to UC Insurance Requirements
When to use this form:
The Request for Exception to UC Insurance Requirements online form is to request a reduction of limits or waiver of the UC Insurance Requirements for uninsured/under insured performers, contractors, vendors or suppliers providing LOW RISK goods or services to UCR.
Ensure that you have gathered all the required information and documentation for the request prior to starting as you are not able to save your information and return to this form.
Required Documents:
- Agreement/Contract/Purchase Order with detailed Scope of Work
- Exception to UC Insurance Requirements: NIA, Waiver of Liability, Assumption of Risk and Indemnity Agreement
- Exception to UC Insurance Requirements: Department Agreement to Assume All Costs
Submit online form with all required documents attached no less than 21 business days before the scheduled event or commencement of contract.
Once the form is completed, the submitter, requesting department contact (if different), and UCR Risk Management will immediately receive an e-mail confirming the submission. It will contain a copy of the information that was submitted.
Review:
Risk Management will review the request within 7 business days and, if additional information is needed, they will return the form for completion or set up a Risk Assessment Meeting to discuss details of the request.
Response:
Once Risk Management’s review is complete, the submitter and requesting department contact (if different) will receive an email with:
- Approval of the full or partial waiver of insurance
- Approval of reduction of insurance limits
- Denial of request with an explanation
Timeline for Request:
Submit request at least 21 business days prior to event or commencement of contract.
Review period is 7 business days from receipt of completed request
If the scope of work changes during the Agreement period, department must submit new request for waiver or obtain the required limits of insurance.
